Possible causes of phimosis include trauma, abnormal development of the foreskin, and genetic predisposition.

Depending on its origin, phimosis is classified as physiological or pathological.

Physiological phimosis is common in newborns and young children due to the natural limited mobility and elasticity of the foreskin. In most cases, the foreskin gradually becomes more retractable as the child grows.

Persistent or acquired narrowing of the foreskin in older children and adult men may be considered pathological phimosis.

Determining the type and severity of phimosis is important for selecting an appropriate treatment strategy and preventing possible complications.

Four grades of phimosis may be distinguished according to the severity of clinical manifestations:

Grade 1. When the penis is flaccid, the foreskin can be retracted freely over the glans. During an erection, retraction requires some effort.

Grade 2. When the penis is flaccid, the glans can be exposed with slight difficulty. During an erection, retraction of the foreskin is no longer possible.

Grade 3. When the penis is flaccid, exposure of the glans is possible only with considerable difficulty. During an erection, the glans cannot be exposed. Difficulty urinating may also occur.

Grade 4. The glans cannot be exposed even partially. Urination is difficult.

Phimosis is a condition in which the glans penis cannot be exposed or can only be exposed with pain or difficulty due to narrowing of the foreskin.
